Key responsabilities:
- Perform targeted case management tasks following best practices
- Engage with patients to track success and identify deviations for intervention
- Assist patients in receiving ancillary services and using the app for services
- Support Case Managers Level 1 and 2, including mentoring and training opportunities
- Work directly with clinicians, pharmacy staff, and manage care schedules
