- Provide targeted case management with patients and support other case managers.
- Track patient success, assist with ancillary services, and support tech usage.
- Work directly with clinicians to coordinate care and manage schedules.
- Train and mentor Case Manager Level 1 and Level 2 staff.
- Assist patients in accessing treatment options both in person and via telehealth.
