Statewide Support Coordinator Specialist

  • Typical working hours: 8:00 AM to 5:00 PM, Monday through Friday.*

Hybrid Work Schedules/Flexible Schedules/No On-call

Currently recruiting for three Support Coordinator (SC) Specialist positions. Two positions will specialize in transitioning individuals moving to the community from State Operated Programs and Facilities. One position will specialize in SC data and training.

Role Description**

State Operated Specialist Positions (2): These positions will serve small, high intensity need caseloads that will include those in DD State Operated Programs and DBH State Operated Facilities transitioning to the community. These positions will provide guidance, training, and coaching to State Support Coordination team members. Additionally, they may support the team in data collection and dashboards, as well as developing support coordination tools and training.

Data and Training Specialist (1): This position will provide guidance, training, and coaching to State Support Coordination teams to ensure that SC teams are providing quality support coordination services. This position will maintain our teams’ data collection and dashboards, as well as develop Support Coordination tools and training.

They also may serve a small, high intensity need caseload. **

Minimum Qualifications**

Bachelor’s degree or Registered Nurse license and 5 years of relevant experience [which includes, but is not limited to, fulfilling the role of Support Coordinator, Support Coordinator III, or Support Coordination Supervisor (or comparable roles within contracted TCM entities)]. **

Knowledge, Skills & Abilities**

Demonstrates the knowledge, skills and abilities outlined in the Missouri Support Coordination Core Competencies . Ability to coach and train staff to meet the Missouri Support Coordination Core Competencies, State Plan Requirements, and Medicaid Waiver Assurances. **

Preferred**

Competency working with data collection systems and/or data interpretation to identify trends.

OR

Demonstrates knowledge of the discharge process from state operated programs and facilities. **

Expectations**

  • Serve as a specialist in statewide support coordination practices
    • Expert in support coordination practices, rules and regulations, and identifying statewide resources.
    • Provide coaching, and necessary training and tools to the Support Coordination Teams.
    • Develop and maintain effective working relationships with internal and external stakeholders.
    • Manage and interpret data related to targeted case management and support coordination.
    • Identify areas of inconsistency and problem solve with team members to develop one path forward.
    • Effectively communicate any updates to processes, policy and/or procedure.
    • Attend regular meetings with State Support Coordinators and Support Coordination Leadership.
    • Present information to stakeholder groups both in person and virtually.
    • Attend 40 hours of professional development and/or training that provides helpful resources and information for the State Support Coordination Teams.
  • Support a small, high intensity need caseload
    • Carry a short term, high support need caseload that will include individuals transitioning to the community from DD State Operated Programs and DBH State Operated Facilities; OR Olmstead, crisis cases (residentially placed and natural home), hospital boarded individuals, individuals with Children’s Division involvement, individuals requesting MoCDD waiver, and/or cases with multiple state agency involvement.
    • Collaborate with sending and receiving Support Coordination teams, other State Agencies, and Provider Agencies to ensure continuity of care as individuals transition to new community living environments.
    • Ensure your caseload documentation meets federal and state requirements.
    • Facilitate meetings as needed.
  • Perform other duties as assigned.

Essential Functions

  • Knowledge of DD/DMH vision, mission, and values.
  • Follow Code of State Regulations (CSRs), Departmental Operating Regulations (DORs), Division Directives and Division Guidelines.
  • Communicate verbally or in writing with individuals, providers, co-workers, and/or supervisors utilizing agency forms, computer programs and email as required.
  • Knowledge of Microsoft Office, Cimor, and Adobe Acrobat. Adaptable to updated technology and programs.
  • Read and follow directions.
  • Must travel to/from individuals home, provider and community locations.
  • Must maintain a valid driver's license.
