Job Overview

Our program helps participants avoid nursing homes and instead, receive the care and services necessary to help them stay safe, comfortable, and healthy living in their own home.

Collaborative medical environment: Be part of an 11 member interdisciplinary team all under one roof in order to provide the best holistic care to our frail elderly population.

The Physical Therapist - PACE position provides services that help restore function, improve mobility, relieve pain and prevent or limit permanent physical disabilities of patients suffering from injuries or disease as they restore, maintain and promote overall fitness and health. Responsible for evaluating individuals with mobility dysfunction and provide a plan of care that is specific to the individual’s goals related to functional independence.

Minimum Requirements

1. Physical Therapist license issued by the State of California Physical Therapy Board required.

2. Prior experience working with the frail elderly (Geriatric) and disabled populations preferred.

3. Current BLS certification also required.

4. Bilingual English/Spanish/Cantonese/Mandarin preferred.
