Our Patient Care Coordinators manage in-home and virtual appointments for providers and clinical staff, utilizing scheduling software to optimize schedules. This role ensures appointments align with patient needs, provider availability, and organizational goals.


KEY DETAILS:

Location: Remote (work from home)

Reports to: Scheduling Operations Manager

Daily work: Conduct outbound calls to schedule in-home and virtual patient visits and continuously optimize schedules.

Collaboration: Works with various team members to manage schedules, maximize productivity, and enhance patient care delivery.


KEY RESPONSIBILITIES:

  • Coordinate and schedule appointments for providers and clinical staff, both in-person (field/home) and virtual visits.
  • Utilize scheduling software to efficiently manage provider and clinical staff schedules.
  • Collaborate with operational and clinical teams to identify and address scheduling gaps, cancellations, or urgent needs.
  • Assist in managing provider and clinical staff availability, time-off requests, and schedule changes.
  • Maintain accurate, up-to-date records of all scheduled visits, cancellations, and reschedules.
  • Communicate with patients regarding appointment confirmations, rescheduling, and other scheduling-related needs.
  • Work with the interdisciplinary team, including engagement specialists and care navigators to ensure a smooth patient experience from initial contact through follow-up care.
  • Support provider and clinical staff productivity targets by optimizing schedules and minimizing idle time.
  • Provide timely reporting and updates to central operations and clinical leadership on scheduling metrics and issues.
  • Ensure that scheduling aligns with organizational standards, provider availability, and patient care requirements.
  • Address patient scheduling grievances promptly and escalate to management when necessary.
  • Participate in team huddles and meetings.
  • Comply with all relevant healthcare laws, regulations, and company standards.
  • Other duties as assigned.

Requirements:

  • High school diploma or GED.
  • 2+ years work experience in a healthcare setting
  • Reside within the state of Washington
  • Preferred: Call center or scheduling experience.
  • Preferred: Experience with patient communication protocols, including handling grievances and service recovery.
  • Preferred: Experience in a value-based healthcare setting, with an understanding of care delivery goals focused on quality and patient outcomes.

Specialized Knowledge and Skills:

  • Proficient in using scheduling software
  • Proficient in electronic health record (EHR) systems.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Knowledge of HIPAA regulations and patient privacy requirements preferred.

Base Salary/ Wage Range $22 to $25 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
