What We’re Looking For:
The Support Transition and Reentry (STAR) -Specialist provides direct support to recipients enrolled in the Trillium STAR care team. The STAR Specialist assists the STAR Coordinator with ensuring the member is supported and assists the coordinator in completing relevant tasks. The STAR Specialist assists to ensure the member is moved to ongoing Care Management services and supports. The STAR Specialist is responsible for working with the recipients care team to provide transition linkages including school, employment, housing coordination, community service opportunities and other identified treatment supports and services. This individual will work with community resources and services to meet the identified needs for recipients who are transitioning back to the community.

Qualifications:
High School, GED
Experience:
One (1) year of experience in the Health and Human Services field, including experience working with any combination of the BH/MH/SU/IDD population.
OR
Be a person with (1) year lived experience with a behavioral health condition and/or substance use.
OR
One (1) year of experience in criminal justice or juvenile justice systems to include justice involved experience.
OR
Equivalent combination of education/experience.
Preferred Experience: (if applicable)
License/Certification Required: None Identified
Preferred:
North Carolina Certified Peer Support Specialists.
Must have a valid driver’s license
Must reside in North Carolina to be considered for remote status: Remote in North Carolina-Within 75 miles of Scotland County
