Overview
Join Medigence Health, a leading provider of Health Risk Assessments, as a Physician or Nurse Practitioner. This pivotal role involves conducting comprehensive assessments for Medicare, Medicaid, and Commercial Marketplace patients across various settings, including home visits, clinics, telemedicine, and long-term care facilities. Key Responsibilities
Conduct detailed evaluations of beneficiaries’ medical and social needs.
Identify and refer patients for routine, urgent, and emergent case management.
Provide recommendations for patients to follow up with their primary care physicians.
Travel within local counties, regions, and/or states where you hold a current license.
Perform comprehensive assessments in provider offices or via Telehealth, ensuring strong audio and video connectivity.
Document findings accurately within the available Electronic Medical Records (EMR) system.
Reside in Connecticut.
Note: Prescribing or altering current treatment plans is not required.
Required Skills
Minimum of 2 years of hands-on experience as a Nurse Practitioner or Physician.
Knowledge of HCC model, HEDIS, and STAR quality measures.
Current, unrestricted licensing in Connecticut or ability to acquire it.
Registration with Medicare is mandatory.
Board certification in a relevant specialty, preferably internal medicine, geriatrics, or family medicine.
Strong communication skills, particularly with elderly and chronically ill patients.
Comfort utilizing telehealth technology and digital tools.
Qualifications
Master's degree in Nursing or Doctor of Medicine.
Active, unrestricted license to practice in Connecticut.
