Job Overview

We are actively seeking a dedicated Community Support Specialist to join our team at Compass Health Network. This full-time position is essential to the delivery of quality and compassionate care within our service areas. In recognition of the critical service needs of the communities we serve, we are excited to offer a sign-on bonus of \(500, with an additional \)500 available for Qualified Mental Health Professionals.

Key Responsibilities

  • Provide individualized and group education services utilizing a motivational, stage-matched approach, focusing on the unique strengths, needs, abilities, and preferences of each client and their family.
  • Collaborate with clients to develop and update Wellness Plans aimed at achieving their recovery and resiliency goals.
  • Assist clients in identifying necessary public services, including financial support, housing, and food resources.
  • Maintain ongoing communication with clients, particularly those in hospitalization, to facilitate discharge planning and aftercare coordination.
  • Deliver education and skill-building in areas such as symptom management, daily living skills, and personal support system development.
  • Engage with integrated health team members to coordinate services and provide comprehensive support to clients.
  • Complete required documentation and follow up on client appointments and services.

Required Skills

  • Exceptional communication and interpersonal skills.
  • Ability to deliver personalized and group educational support.
  • Knowledge of mental health and substance abuse treatment methodologies.
  • Strong organizational skills for documentation and client follow-up.
  • Proficiency in working collaboratively within an integrated health team.

Qualifications

  • High School diploma or GED is required; an Associate's or Bachelor's degree in a human service-related field is preferred.
  • Relevant experience working with individuals who have psychiatric, substance use, or developmental disabilities is necessary.
  • A valid driver's license and agency-specified minimum automobile coverage are required.
  • Experience requirements vary based on educational attainment, with a minimum of one to four years of relevant experience needed.
