Job description

Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

The Clinical Manager, Nurse Practitioner or Physician Asst will oversee and support our provider network. This position will be responsible for ensuring the quality of care provided by our clinicians for all visit types, fostering a supportive and productive work environment while enhancing clinician performance.
Extensive clinical experience, leadership skills, and a passion for mentoring and developing clinicians. This role plays a key role in interviewing, and orienting new clinicians, conducting performance reviews, ensuring quality documentation, and providing education and support to help our clinicians achieve excellence.

Responsibilities include:

  • Oversee clinical network of providers.
  • Assist the National Clinical Practice Director with assuring adherence to all policies and procedures of both self and clinician network.
  • Interviewing & Orientation: Interviewing new candidates and coordinating orientation to ensure a smooth onboarding. Work with our team to get all supplies and IT equipment needed.
  • Annual Reviews and Performance Management: Conduct annual performance reviews for all clinicians, providing constructive feedback, setting goals for professional development, and implementing action plans to address performance gaps.
  • Monthly Documentation Reviews Review clinician documentation monthly and as needed for accuracy and completeness, providing feedback and training as needed.
  • Monthly KPI’s: Monthly KPIs according to areas decided upon by Director of Clinical Practice. Track and report on monthly Key Performance Indicators (KPIs) related to clinician performance, identifying trends and areas of improvement and collaboration with team to meet goals. 
  • Payroll Assistance: Approval of Part-Time clinician hours worked.
  • Monthly Clinician Education Meetings: Assisting the Director of Clinical Practice to plan and facilitate monthly education meetings for clinicians focused on best practices, clinical updates, and professional development.
  • Weekly Open Office Hours: Hold regular weekly open office hours to provide ongoing support to clinicians, answer questions, and address any concerns in a collaborative and approachable manner.
  • Supervises/Manages Others (i.e. hires, performance reviews, corrective action, etc.)
  • Other duties as assigned.
  • All other duties as assigned.

Qualifications and experience:

  • Graduate of an Accredited Physician Assistant program or Nurse Practitioner program required.
  • Minimum 5 years of experience practicing as a Nurse Practitioner or Physician Assistant.
  • Minimum of 3 years of experience in a supervisory role.
  • Active California NP or PA license

  • Current Board Certification

  • Strong organizational skills: Ability to multitask and manage a wide variety of responsibilities in a fast-paced environment.
  • Ability to appropriately maintain confidentiality and handle sensitive information with discretion. Strong knowledge of Risk Adjustment and HEDIS (Healthcare Effectiveness Data and Information Set) measures.
  • Strong interpersonal skills, including the ability to build rapport with clinicians and other team leaders. 
  • Knowledge of clinical best practices.
  • Experience with performance tracking.
  • Ability to work independently and collaboratively in a team environment.
  • Knowledge of eClinicalWorks (eCW) EMR.
