Posting Salary Range: $53,045 - $66,837

Trillium is seeking a passionate and dedicated Practice Management Consultant to play a vital role in shaping the future of employment services for individuals with serious mental illness (SMI) and severe and persistent mental illness (SPMI). As a Practice Management Consultant of Member Employment Services, you will be at the heart of supporting and developing Trillium's Individual Placement and Supports provider network. Your primary focus will be to ensure that these providers deliver high-quality services that adhere to the established fidelity model. Why is this so important? Because consistent, model-driven services lead to positive outcomes, specifically, helping individuals with SMI and SPMI achieve and maintain competitive integrated employment (CIE).

On a typical day, you might:

  • Develop and maintain knowledge of IPS Fidelity Model to reinforce fidelity with providers.
  • Represent Trillium in the Provider Fidelity Review process and maintain information related to fidelity reviews including dates of reviews, provider outcomes and scores.
  • Track data and identifies systemic needs to improve fidelity.
  • Develop plans to support Provider Network on areas of deficiency.
  • Complete Benefits Counselor training and share guidance from the State with the IPS teams related to Benefits Counseling.
  • Provide consistent and accurate information to the provider network that de-bunks myths about how work impacts benefits.  
  • Monitor, coordinate and provide Technical Assistance (TA) related to NC Core Milestone Payment Model.
  • Facilitate routine monitoring of NC Core to ensure Milestones have been accomplished and documented accurately to support Milestones billed.
  • Assess for and building IPS capacity and utilization.
  • Collect caseload data from providers.
  • Identify when a provider is at capacity and determine if a new Employment Specialist needs to be added or if Trillium needs to consider adding a new team.

Qualifications: 
 

Required Education/Experience:
Associate’s degree in Human Services, Health Administration, Business Administration, or Public Health Administration field and four (4) years of experience with mental health, substance use, and/or intellectual/developmental disability populations (operations and/or member-facing), or experience in healthcare operations, member-facing or policy experience. Experience with data analysis and use of reporting tools required. Working knowledge of value-based purchasing, value-based care and/or alternative payment model concepts.

OR 

Bachelor’s degree in Human Services, Health Administration, Business Administration, or Public Health Administration field and two (2) years of experience with mental health, substance use, and/or intellectual/developmental disability populations (operations and/or member-facing), or experience in healthcare operations, member-facing or policy experience. Experience with data analysis and use of reporting tools required. Working knowledge of value-based purchasing, value-based care and/or alternative payment model concepts.

OR 

Equivalent combination of higher education/experience. 

 

Preferred Education:
 
Master’s degree in Human Services, Health, Business Administration or Public Health Administration.

Preferred Experience:
Experience in a managed care organization / environment or healthcare setting preferred. Knowledge of population health management, medical expenses and quality performance analysis preferred. Working knowledge of relevant evidence-based and best practices preferred. Proficient in standard office software, and project management tools preferred.

 

Required License/Certification: 

Must have a valid driver’s license
