Primary Duties & Responsibilities:
Completes intake assessments with patients referred to the CONNECT Program and others referred for psychotherapy services in the Division of Child and Adolescent Psychiatry (CAP) for outpatient adolescent behavioral healthcare.
Provides screening and psychoeducation.
Triages patients into the appropriate program, provider, or provide referral to off campus providers.
Schedules psychotherapy and psychiatry services for patients as indicated.
Manages patient scheduling services to include collecting demographics and insurance registration.
Maintains accurate and timely documentation in electronic medical records (EMR).
Provides follow-up communication as needed.
Tracks programmatic data and prepares outcome reports.
Attends team meetings and participates in training sessions as assigned.
Builds relationships with on-campus colleagues and community partners to ensure collaborative continuity for start of care.
Provides referrals to community services as needed.
