About the Role
An attractive position is currently available for an experienced allied health or nursing professional to extend skillset and work as a Team Leader with the Transition Care Program based at St George’s Health Service in Kew.
The Team Leader position holds a clinical caseload and leadership responsibilities. As part of the leadership team you will responsible for supervision/support of other staff, education, training, staff development, service development, quality improvement and ensuring that the program continues to provide high quality, evidence based clinical services that lead to improved health outcomes for clients.
Duties
Provision of high quality and evidence-based patient centred care.
Provide leadership support with day to day operational functions including program intake, occupancy management, leading case review, team meetings and managing staff performance including leave
Deliver and/or participate in supervision, professional development and performance review process
Act as a resource for staff within an identified area of expertise and provide secondary consultation to staff across TCP
Lead quality and continuous improvement initiatives, team processes and service development activities
The classifications for this position will be Allied Health Grade 3 ($102,091 - $116,646 per annum) or Nursing Clinical Consultant C ZA7 ($125,288 per annum).
Your Contribution
Tertiary Allied Health or Registered Nurse qualification.
Minimum of 7 years’ experience in a health care setting
Experience leading an interdisciplinary team
Demonstrated ability to effectively work with relevant stakeholders to achieve best outcomes for clients
Proven ability to effectively manage and negotiate solutions to complex problems
Highly developed interpersonal skills that incorporate the qualities of excellent communication, negotiation, organisation, planning and problem solving
Current national criminal history check, or willing to obtain

About Transition Care Program
Transition Care services target older people at the conclusion of a hospital admission who require more time and support to complete their restorative process, optimise their functional capacity and finalise and access their longer term care arrangements.
The team includes case managers, physiotherapists, occupational therapists, podiatrists, dietician, speech pathology, allied health assistants and administrative support.
