The primary location for this role is remote, travel is estimated to be up to 40%, and expected schedule is full-time (40 hours) 4 days per week. Active Texas, Florida, and New York Licensure is required.
What You’ll Do
As a Launch Advanced Practice Provider at Imagine Pediatrics you will report to the Medical Director, Launch to support our mission of increasing safe days at home for medically complex children. This role is integral to our success as an organization and will serve as a utility team to launch new markets and support existing ones.
In this role you will:
- Manage a panel of complex pediatric patients, providing comprehensive oversight of their care plans and coordinating care across the healthcare system using a virtual platform.
- Collaborate with pediatric RNs and other care team members, offering guidance and resources to support complex pediatric patients.
- Educate and empower parents by providing tailored education and reinforcement of care plans to ensure comprehensive support within the home environment.
- Leverage virtual platforms to conduct patient visits, ensuring care is accessible, timely, and patient-centered.
- Partner with a multidisciplinary team including nurses, physicians, behavioral health therapists, dietitians, pharmacists, and mobile clinicians to deliver coordinated and optimized care.
- Support new market launches by serving as a foundational anchor team during go-live phases.
- Step in as needed to provide additional support to existing markets, ensuring continuity and high standards of care.
What You Bring & How You Qualify
First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with complex medical conditions gets the care and support they deserve.
- Minimum 5 years’ experience with a pediatric population required, Primary Care treating complex care, Pediatric Complex Care, or PICU.
- You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license in Texas, New York, and Florida, including board certification
- Medicaid enrollment in New York and/or Texas is highly preferred
- Willingness to become cross licensed in new states as requested based on business needs
