Development and Alumni Relations (DAR) supports Johns Hopkins’ focus on research, teaching and patient care, and its role as a national and global leader in higher education. We create and foster enduring relationships that result in advocacy and philanthropic support for Johns Hopkins University & Medicine; strengthening the institution through partnerships with donors, alumni, volunteer leaders, faculty, students, staff, and patients.


The Fund for Johns Hopkins Medicine (FJHM) is the dedicated development and alumni relations team supporting the entities that comprise Johns Hopkins Medicine (JHM), including clinical departments, hospitals, satellite clinics, and the School of Medicine. We contribute to Johns Hopkins Medicine’s mission of patient care, research, and education through securing philanthropy, building and maintaining donor and alumni relationships, and supporting institutional priorities. FJHM partners closely with institutional leaders, faculty, and clinicians to engage alumni, patients, donors, and prospects in JHM’s work to advance medicine and improve the human condition.


The Clinical Referral Navigator is strategically positioned clinical practitioner and user of systems that impact patients' care. They function as the clinical member of a highly skilled team based in FJHM managing navigation for clinical referrals from Trustees and donors across JHU.


The Navigator will have experience in patient relations and in referral management. They are a clinical resource and advocate with the overriding goal of ensuring timely, seamless and efficient access for new patients and referring physicians across Johns Hopkins Medicine (JHM).


The Navigator’s main area of focus is to improve access and new patient experience – ensuring the delivery of patient centered care and, when appropriate, providing patient education.


The Navigator will coordinate patient care through the care continuum - starting with initial point of entry through discharge (as needed). They will serve as a centrally-based referral expert for the efficient coordination of multi-subspecialty appointments across the JHM system and is a liaison to community resources.


Key responsibilities: 

  • Ensure timely access to appointment scheduling for new patients. Circumvent barriers, as needed;
  • Develop scheduling workflows and protocols to facilitate appointments across ambulatory sites at JHM;
  • Ensure timeliness of new patient appointments across JHM departments;
  • Serve as a centrally-based primary point of contact for developing patient appointment flow for complex multi-subspecialty patients into JHM;
  • Partner with JHM’s faculty and administrative stakeholders to create comprehensive care plans for complex patients who require diagnostic testing and multiple consultations;
  • Assist in obtaining records, ancillary testing in advance of new patient appointments;
  • Navigate testing and appointments, as needed;
  • Develop network of preferred referring providers and facilitate appointment scheduling to other specialists, as needed;
  • Develop processes and workflows to ensure timely follow up communication with referring providers;
  • Support patients in utilizing technology (i.e., MyChart) to facilitate care and communication;
  • Report on key metrics including patient experience, number of patients served, referral sources and average time to appointment, and;
  • Manage navigation for a portfolio referrals (N= 35), based on department-specific procedures and to maintain clinical safety and quality.
  • Foster and maintain working relationships with all appropriate medical staff.
  • Build and support a varied network of individual physicians to help manage the care plans for the patient.
  • Serve as a primary JHM point of contact for new patients and high acuity patients.


Minimum Qualifications
  • Bachelor's degree in nursing.
  • Current RN Maryland licensure.
  • Three years related experience in healthcare setting.

Starting Salary Range: Min: $62,899 - Max: $110,100 (targeted salary: $90,000 annualized, commensurate with experience)
