Posting Salary Range: $62,712 - $77,654

Office Location: Remote within County. Must reside in or within 75 miles of Scotland County, North Carolina.

What We’re Looking For

Trillium Health Resources has a career opening for a Support Transition and Reentry (STAR) Clinician to join our Support Transition and Reentry Care Team. This program assists adults existing a Department of Corrections or County Detention facility who also experience behavioral health or Intellectual and developmental disabilities (IDD) conditions. The primary responsibilities of this position are to ensure STAR recipients are well linked and able to access needed treatment and support, to disability benefits, employment, housing coordination, community service opportunities and other identified treatment services. This individual will work with community resources and services to meet the identified needs for recipients who are transitioning back to the community.  

On a typical day, you might:

  • Collaborate with STAR Coordinators and Specialists on systems and processes needed to assist with successful Community re-entry.
  • Coordinate and manage care for members who are participants of the Transitions to Community Living program.
     
  • Perform clinical functions of discharge/transition planning and diversion including clinical interview; obtain and review clinical records; identify treatment needs; assess barriers to treatment and recommend solutions; and assess general health and specific behavioral health needs and recommend referrals.
     
  • Ensure Transition Plans are delivered in the most effective manner with a focus on optimal quality outcomes utilizing evidence-based tools and national standards of care.
     
  • Review clinical documentation such as comprehensive clinical assessments and psychological evaluations to ensure appropriate recommendations for services and level of care.
     
  • Meet with Justice Involved members in-person in Correctional Facilities and in-person with the members in the community to facilitate care management activities.
     
  • Assist participants with transportation needs. May transport members in fleet vehicles when available to community-based settings as part of development of community reintegration planning.

Qualifications: 

Required Education: 
 A Master’s degree

Preferred Education:
 Degree in Human Services or Criminal Justice

Required Experience: 
 
A minimum of one (1) year of relevant experience working directly with individuals with Serious Mental Illness (SMI); or must have one (1) year prior relevant and direct experience providing diversion services under TCLI.


 
Preferred Experience:
Minimum of one (1) year working in criminal justice or juvenile justice systems.

Minimum of two (2) years post-Master’s experience in a clinical setting with the MH/SU population.

Prefer experience working with individuals with severe and persistent mental illness who are living in an institutionalized setting.


 Required License/Certification

Fully licensed by the state of North Carolina as a LCSW, LPA, LCMHC, LMFT, or LCAS is required.
Must have a valid driver’s license
