The Opportunity  

 

We are looking for 2 experienced Registered Nurses to join our Clinical Programs team as Care Coordinators to support the RITH program that Amplar Health delivers to patients in their home. We have 2 opportunities available, one being a 12 month parental leave contract, working 3 days a week and the other is a permanent position working 4 days a week. Whilst both roles have the option of being fully remote i.e. work from home, we are looking for candidates based in Victoria and who have a strong understanding of the VIC healthcare system and services.

 

Reporting to the Program Lead, you will initially be trained to support our RITH program but, over time, may also be upskilled to support the HITH and AHHC programs. Using your clinical skills, you will ensure a comprehensive patient centred management plan is achieved by:

 

  • Conducting initial phone assessment, care planning and goal setting.
  • Coordination of in-home services to support the rehabilitation and recovery of patients.
  • Monitoring patient progress and updating care plans as appropriate.
  • Building and maintaining effective relationships with a variety of different stakeholders including primary and secondary care organisations and your non-clinical support team.
  • Liaison with healthcare professionals and specialists.
  • Provision of telephone support, education, and advice.

 

    About You

 

As an outstanding communicator, your intuitive people skills will be central to your success. You will be naturally empathetic, observant, attentive, trustworthy, and committed to making your patient feel safe and comfortable. You will be client and goal focused who is driven to achieve positive customer outcomes. You will also have:

 

  • Minimum 2 years' experience as a Registered Nurse with unrestricted AHPRA registration
  • Experience working in acute setting and ideally experience in care coordination.
  • Strong customer service orientation and relationship management.
  • Excellent clinical reasoning skills.
  • Ability to work in a fast-paced environment.
  • Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
  • Have a reliable internet connection speed.
