Job Summary:

The Family Facilitator works in close collaboration with the PA Office of Developmental Programs (ODP), PA Office of Medical Assistance Programs (OMAP), Managed Care Organizations (MCOs), healthcare organizations, PCCRC (Pediatric Complex Care Resource Center) staff, families, and community service providers to coordinate and ensure smooth transitions for children with medical complexities moving from hospitals or congregate care settings to home-based living. This role acts as a team lead adept at planning, coordinating, and executing safe discharge plans that address the unique needs of each child. This role also actively participates in supporting the diversion of child placement from community to congregate care settings.

Job Duties:

  • Work closely with Health Care Systems, Managed Care Organizations (MCO), Department of Human Services (DHS) offices to accept referrals of children with medical complexities in need of permanency with a family or family like or community-based setting

  • Facilitator partners with ODP and ensures that referrals to qualified Medically Complex Conditions providers have been submitted.

  • Facilitator works with families and County Administrative Entity Offices for registration and waiver enrollment.

  • Build relationships that engage patients, families, caregivers, and healthcare providers

  • Organize meetings with the family and key members of the healthcare team to discuss discharge planning and share family’s vision.

  • Conduct outreach to families, community health related/promotional organizations, hospital/facility social workers, and discharge planners regarding in-home services and options for family-based alternatives to institutional care

  • Facilitator ensures that the family and child have the necessary in-home supports to ensure a sustainable/safe discharge.

  • Facilitator collaborates with MCO Special Needs Unit case managers to ensure that the child has needed approvals for in home nursing, durable medical equipment, and home adaptations in place at time of discharge.

  • Visit facilities and hospitals to meet children, families and
    discharge support team

  • Act as a transitional coordinator until the child has permanency

  • Provides information and facilitate linkages to community supports, services, providers, service coordinators and advocacy resources to families

  • Facilitator will clearly document work performed.

  • Facilitators will complete required data collection per OMAP provided templates

Position Details:

  • Hours:  8am-4:30pm

  • On-call: Requires some flexibility to alter standard work hours slightly to meet the need of the program.

  • Travel: More than 50% - Requires travel to different facilities/hospitals/homes/homes

  • Required Experience: 5 years of related work experience

  • Preferred Experience: MSW Preferred, Bachelors Social Work required.  Five years experience in Human Services field, experience with children with medical complexities, Intellectual Disabilities systems knowledge preferred.
