Job Overview

Join Medigence Health as a Physician focused on conducting Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace patients. This role involves performing thorough assessments that adhere to annual wellness visit guidelines, all within the comfort of patients’ homes. Key Responsibilities

  • Conduct comprehensive evaluations of the medical and social needs of beneficiaries utilizing Medigence Health’s tablet-based assessment tool.
  • Identify and refer patients for routine, urgent, and emergent case management as necessary.
  • Provide recommendations for follow-up care with primary care physicians.
  • Travel across various counties and regions where you hold a valid license in Wisconsin.
  • Willingness to engage with patients in their homes or via telehealth technology, delivering care in a comfortable setting.

Required Skills

  • Minimum of 2 years of hands-on experience as a Physician.
  • Proficient understanding of HCC model, HEDIS, and STAR quality measures.
  • Current and unrestricted medical license in Wisconsin or the ability to obtain one.
  • Registered with Medicare and Wisconsin Medicaid.
  • Board certification, preferably in internal medicine, geriatric, or family medicine.
  • Strong communication skills, particularly with elderly and chronically ill patients.
  • Comfort in utilizing computer, tablet, and mobile technologies.

Qualifications

  • Must hold a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
  • Proven expertise in effectively communicating with elderly and chronically ill members.
