Job description

Job Overview

Medigence Health is urgently seeking dedicated Physicians and Nurse Practitioners to conduct Health Risk Assessments for our Medicare, Medicaid, and Commercial Marketplace members. This role is pivotal in enhancing the quality of life for our patients through comprehensive evaluations that comply with annual wellness visit standards. The position allows for flexible working conditions, including remote assessments, and requires travel within Wisconsin.

Key Responsibilities

  • Perform thorough medical and social assessments of beneficiary members using our tablet-based assessment tool.
  • Identify and refer patients for routine, urgent, and emergent case management as needed.
  • Provide recommendations for follow-up care with primary care physicians.
  • Engage with patients in various settings such as homes, clinics, or through telemedicine.
  • Travel in and around designated counties in Wisconsin, including Dane, Kenosha, Milwaukee, and others, where you hold a valid license.
  • No prescribing or modification of existing treatment plans required.

Required Skills

  • Minimum of 2 years of hands-on experience as a Physician or Nurse Practitioner.
  • Familiarity with HCC model, HEDIS, and STAR quality measures.
  • Current and unrestricted state license or the ability to obtain one.
  • Registration with Medicare and Wisconsin Medicaid is essential.
  • Board certification in internal medicine, geriatrics, or family medicine specialty is preferred.
  • Excellent communication skills, particularly in relating to elderly and chronically ill individuals.
  • Comfortable with home visits and proficient in telehealth technology.

Qualifications

  • A Master’s degree in Nursing or a related field is required.
  • Advanced Practice Registered Nurse (APRN) credentials are necessary.
  • Strong interpersonal skills for effective patient interactions.
