By acting as the first point of contact for the justice systems and programs with whom we partner, Holon Health provides timely and efficient brief interventions to better prepare patients to receive community-based services and engage in proactive, prosocial behaviors.


POSITION SUMMARY:

Holon Health is seeking a part-time, hourly Nurse Practitioner to support our Collaborative Care Management (CoCM) efforts. The NP will work collaboratively with the care management team to ensure optimal patient outcomes by reviewing patient medical records, identifying and addressing potential care gaps, and facilitating timely and appropriate transitions of care. This role does not involve direct patient interaction.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct comprehensive medical record reviews: Analyze patient medical records, including provider notes, lab results, imaging studies, and discharge summaries, to identify potential care gaps, clinical complications, and areas for improvement in the plan of care.
  • Identify and address potential care barriers: Collaborate with members of the clinical team to address potential barriers to care, such as access to services, medication adherence, and social determinants of health.
  • Facilitate timely and appropriate transitions of care: Assist in coordinating discharge planning, including identifying post-acute care options, arranging for home health services, and ensuring the safe and timely transfer of patients to the appropriate level of care.
  • Utilize clinical judgment and expertise: Apply advanced nursing knowledge and critical thinking skills to assess patient needs, identify potential risks, and develop appropriate care plans.
  • Maintain accurate and timely documentation: Document all clinical assessments, interventions, and care plan updates in the electronic medical record system.
  • Participate in quality improvement activities: Collaborate with the care management team to identify opportunities for improvement in care processes and outcomes.
  • Stay current on relevant clinical guidelines and best practices: Maintain knowledge of current evidence-based practices, regulatory requirements, and industry standards related to care management and utilization review.
Requirements

QUALIFICATIONS:

  • Active and unrestricted Arizona license as a Nurse Practitioner (additional state licenses are a plus)
  • PMHNP/PMH-APRN preferred
  • Several years of experience in the practice of addiction medicine or psychiatry
  • Board certification in area of specialty preferred
  • Strong affinity with and compassion for people with SUD
