Job Description
A bit about this role:
The Renal Social Work Case Manager provides social work support to our Chronic Kidney Disease Program within the Specialty Care virtual clinics. This is a new program and this role will be instrumental in driving forward innovations in kidney care at Devoted.
This social work role will complement our interdisciplinary team by addressing the psychosocial needs of our patients. This means advising on the social and practical implications of treatment decisions, and helping patients navigate healthcare systems including connection to community nephrology providers. This role will assist our interdisciplinary team in teaching patients about kidney disease, dialysis, and kidney transplants and connect patients to community resources and support networks while helping patients adjust to treatment plans and diet changes.
We are on a mission to replicate best-in-class Chronic Kidney Disease care through our virtual care platform. Our multidisciplinary team combines remote monitoring, automated medication management, and high-touch disease education to deliver seamless speciality care in the comfort of our members’ homes.
The goal of launching this new program is that we seek to better serve our members who often lack access to high-quality Chronic Kidney Disease care because of a scarcity of specialists in their community or personal barriers to traveling to seek care.
We accomplish this by providing members with the knowledge, treatment, and support required for members to become self-managers of their illness.
Required skills and experience:
Master’s degree in social work and active license with the willingness to gain additional state licenses
Minimum of five (5) years expertise in the medical field
Minimum three (3) years in renal care
Desired skills and experience:
LCSW or LMSW or equivalent clinical license in social work
Bilingual in English and Creole, Spanish, or Vietnamese
NSW-C or other renal social work certification
Your Responsibilities and Impact will include:
Contributing to role development in a start-up program as the first renal social work case manager on the team
Telephonic and video visits with patients and caregivers
Establishing trust and building strong relationships with our patients
Interdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patients
Conduct interventions to maximize the effectiveness of the program: Create personalized care plans tailored to members’ needs; Assist in navigating family/support issues; Identifying and intervening on social determinants of health via resource acquisition and provision; Place/follow through on behavioral health referrals; Diet and lifestyle education; Advanced care planning
Balancing multiple competing priorities from patients
Working with family members/caregivers to help them in understanding the member’s needs and how they can be supportive
Facilitating and participating in joint visits
Participating in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development
Building relationships and coordinating with key local market and internal stakeholders
Documenting all clinical interactions in the electronic health record
Salary Range: $55,000 - $79,000 / year
Our ranges are purposefully broad to allow for growth within the role over time. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
