Description

Position Summary:

 

Working under a grant from the Office of Refugee Resettlement (ORR), NYAP’s Post Release Services (PRS) Intensive Case Manager Level 3 provides post release services focused on stabilizing the family unit utilizing trauma-informed and intensive case management. The PRS Intensive Case Manager Level 3 will additionally connect with community-based services to address concerns that require long-term care. This position is required to travel to the initial in-home assessment followed by weekly in-person contacts for the first 45 to 60 days, then monthly in-person or virtual contacts for 6 months, depending on the needs of the family.

 

Responsibilities:

 

  • Provide Level Three (3) post release services.
  • As needed PRS Intensive Case Managers may also provide Level Two (2) and Level (1) post release services.
  • Ensure the delivery of intensive in-home services and engagement with children and family.
  • Connect the released child and sponsor family with community-based services.
  • Maintain monthly in-person or virtual contacts for 6 months following referral acceptance.
  • Carry a minimum caseload of 3 cases per worker.
  • Establish working relationships with community-based organizations.
  • Have a case management approach that assesses, plans, facilitates, and advocates for unaccompanied children and their sponsors using knowledge of social services and human services.
  • Follow all ORR guidelines and policy requirements.
  • Demonstrate working knowledge and understanding of ORR Policy and Procedures, Flores Agreement as well as other applicable, and pertinent regulations and laws including the Trafficking Victims Protection Reauthorization Act (TVPRA) of 2008 et.al.
  • Reports to PRS Clinical Supervisor.
  • Perform other duties as assigned.

 

MINIMUM QUALIFICATIONS

 

  • Must be at least 21 years old.
  • Master’s degree in social work, psychology, sociology, or other relevant behavioral science in which direct clinical experience is a program requirement or bachelor’s degree with at least 5 years of clinical experience. May be licensed, or eligible for licensure.
  • Bilingual in English and Spanish.
  • Prior experience with ORR HS/PRS preferred.
  • Proficient in the use of ORR’s Official System of Record (also known as the UC Portal). 
  • Ability to travel up to 75% of the time with motor vehicle license, reliable automobile, a good driving record, and 100/300 insurance coverage on personally owned vehicle.
  • Pass a criminal history screen, including state and local child protection agency registries, and C/ANs check.
  • Willingness to work flexible and non-traditional hours in the service of persons served.
  • Must live within a 100-mile radius of the city they will be focused on supporting.
  • Travel required to states within assigned region.
