The RN-Transitional Care Nurse will use extensive knowledge and experience to identify Medicaid-eligible recipients (patients) with healthcare needs and support them in transitioning from inpatient to home or community-based settings. This includes providing non-clinical services, delivering relevant education on patient health conditions, ensuring proper post-discharge follow-up, and linking them with necessary community resources. These services may be provided in the health care setting, at home, via phone, and/or in person. This position will travel to locations within the relevant service area through a reliable means of transportation insured in accordance with Company policy. This position has occasional work-from-home opportunities. 

REQUIRED:

  • BSN, ADN, or Diploma of Nursing
  • BSN requires experience in a hospital or home health setting
  • ADN or Diploma of Nursing requires two years of direct patient care experience within the last three years in a healthcare setting (preferably hospital or home healthcare agency) 
  • Current RN license in good standing with the State of Alabama Board of Nursing
  • Valid driver’s license in good standing
  • May require significant face-to-face recipient contact with duties performed away from the principal place of business 
  • Willing to submit to vaccine testing and screening
  • Excellent interview and telephone skills as well as the ability to work with recipients in a caring and helpful (assertive and tactful) manner
  • Working knowledge of health-related service delivery systems
  • Excellent communication and relationship skills
  • Ability to analyze varied environmental factors in relation to the recipient's well being
  • Organization and time management skills
  • Ability to be flexible, adaptable, and to work effectively in a variety of settings
  • Basic computer skills, including Microsoft Word and Excel

PREFERRED:

  • Certified Case Manager (CCM)
