Job Description Summary:

This position is responsible for providing education and support to screening and patients. Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. This position exists in various departments throughout OhioHealth system of hospitals and healthcare services.

Minimum Qualifications:

High School or GED (Required)

Applicable public health / addiction treatment knowledge expertise.Desirable certification may vary based on the department in which the job exists.
