Description:
Position Purpose:
The Care Transitions Case Manager collaborates with physicians, social services, nurses, and community agencies to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Care Transitions Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition of patients through medically appropriate levels of care.
What you will Do:
Performs a staff case manager role primarily in the Emergency Department.
Works with complex Emergency Department patients in order to facilitate appropriate care coordination for those patients discharged from the ED.
Identifies and evaluates Emergency Department patients who may require admission and consult with providers and other clinicians regarding appropriate status for those patients.
Works with Director of Case Management and others to develop tools and system and process revisions needed to develop ED Case Management role and facilitate smooth transitions for patients from the ED to other settings.
Hours/Schedule:
Part time; Monday - Friday Rotating days 2-12 hour shifts per week.
Minimum Qualifications:
Education
1. Registered nurse, licensed in the State of Iowa.
2. Bachelor’s Degree preferred or willing to obtain.
3. Case Management certification preferred.
