About the role
Liaising with injured workers, our providers, and internal and external stakeholders to assess and support injured workers to improve job readiness, achieve optimal outcomes and facilitate new employment.
To be successful in this position, you will need a natural ability to proactively establish, maintain and grow relationships with clients, providers and stakeholders; you will be passionate about helping a diverse range of people to enhance their lives and be capable to adhering to Policy and procedures and you will have the ability to use creativity and initiative when sourcing employment opportunities for clients.
- Facilitate assessments, reviews, develop plans and strategies to define barriers and capacities, strengths and provide additional support for the trajectory for the injured worker
- Maintain a caseload of clients
- Planning, implementing and evaluating programs which are designed to meet individual needs
- Support participants to actively utilise and extract value from health and wellbeing application that supports their personal goals
- Work closely with DXC RTW case managers to ensure smooth handovers of support and consistent receipt of benefits (where appropriate)
- Work closely with participants, their families and health/community services to oversee and facilitate appropriate engagement, assessment, and receipt of health, community, and welfare support
Applicants must have:
- The ability to gather information from a range of stakeholders to develop effective case management strategies
- Experience in case management to effectively facilitate outcomes
- Well-developed problem solving and decision-making skills
- Negotiation and influencing skills
- Knowledge of medical and paramedical terminology and procedures,
including the forms of treatment applicable to injuries - The ability to provide clinical service delivery leadership with capacity to assess needs and provide interventions across a range of life areas and needs
- Proven conflict resolution skills and an ability to diffuse potential situations
- Strong written and verbal communication skills
- Flexibility and ability to adapt to change
- Accurately identifying
- Tertiary or relevant qualifications in health/social services desirable
- Ability to work with different IT applications
- Understanding of challenges for individuals re-engaging with employment markets and labor market requirements
