Summary of Job Duties:
This position is responsible for providing continuous quality improvement, project management, performance oversight, and the quality outcomes of population health and all value-based care efforts for primary care clinic patients in the Primary Care Population Health Service Line (PCPHSL) under the direction of the Quality Director for Population Health and Value Based Care. The Population Health Improvement Manager (PHIM) interfaces with the multidisciplinary leadership team including the clinic’s access, nursing, physician, and administrative leadership to ensure quality and performance metrics are clearly defined and necessary workflows, interventions, and standard work practices are in place and executed. This position will work out of Little Rock and Pine Bluff Clinics.

Qualifications:

Minimum Qualifications:

  • Baccalaureate degree in one of the following areas:  Health Information Management, Health Education, Public Health, Nursing, or healthcare related Field.  Equivalent combination of experience and/or education may be substituted in lieu of a Bachelor’s degree
  • Plus four (4) years’ experience in quality improvement, performance improvement, clinical work, or program management and project management
  • Must have excellent written and verbal communication skills.
  • Knowledge and experience related to performance improvement and change management.  Knowledge and experience related to accreditation and regulatory compliance. 
  • Knowledge of Microsoft Office (Excel, Word, PowerPoint), Clinical experience using an electronic medical record system. Detail oriented, with ability to work as part of a team and independently.

The PHIM provides oversight of site visits, manages quality plans, coordinates with the PCPHSL to addresses Quality, Patient Experience, and Safety outcomes.  PHIM will report and address ongoing performance projects to improve and hold gains, help inform best practices, and support educational efforts.  When appropriate, the PHIM will provide supervision to Population Health Improvement Assistant(s).

Communication for Quality Performance - Conduct Meetings, Creates Visual Display, and Education
  • Maintain knowledge of payer value-based programs, population health innovation, and quality improvement methodology and ensure this knowledge is communicated and operationalized to support clinic leaders in meeting the goals of the clinic, the IMSL, and value-based programs as it relates to quality, safety, and patient experience.
  • Attends and/or supports the facilitation of regular clinic meetings and huddles with multi-disciplinary leaders to establish goals, improvement projects, and operational activities and will be accountable for reporting out progress.
  • Participate in educational activities that engage physician, residents, and staff and promotes understanding of Patient Centered Medical Home concepts, value-based care models, IMSL best practices, and quality improvement processes and techniques. Will attend approved trainings and calls as directed by the IMSL Quality Director.        
  • Facilitates meetings and collaborations with Patient/Family groups and community services to improve patient experience and community continuum of care of services and supports.
  • Utilize appropriate visual graphical tools and data displays to provide interpretation ease and meaningful conclusions.

Data Review, Reporting, and Analysis
  • Conducts focused data analysis to determine causal factors and gaps, or special causal variation related to key performance targets and measures.
  • Compiles data and narrative reports that summarizing findings, determine causes, and make recommendations for improvement.
  • Compiles reports and presentations that summarize improvement projects and successful improvement activities. 
  • Designs and implement chart audit and quality monitoring tools as necessary.
  • Serves as a subject matter expert regarding elements needed to meet quality reporting measures and metrics requirements.
  • Utilize the Epic and Power BI reports, along with payer related portals and reports to identify trends, outliers, and develop improvement strategies and track progress
  • Review and attest for various quality improvement or value-based initiatives.
