Job Summary:
Works within the community providing outreach to patients with complex medical and social needs. Acts as a resource to assist patients in navigating the health care system.
Job Duties:
- Assists in the identification and outreach of medically complex patients.
- Collaborates with the multidisciplinary team in the development and implementation of the care plan.
- Engages patients by way of phone call, home visit, hospital visit and accompanying patients to Providers appointments as needed.
- Completes vital signs.
- Identifies gaps in care and connects members with resources.
- Facilitates telemedicine visits with members and providers.
- Performs disease specific education.
- Conducts Medication Review to ensure medication adherence.
- Conducts safety evaluations in patients home.
- Facilitates adherence to treatment plan by early identification and resolution of gaps in care.
- Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
- Provides reinforcement of an established treatment plan under the direction of the Case Manager and Licensed Social Worker.
- Schedules appointments and performs clerical duties as needed.
- Builds relationships within the community in order to identify resources that are available to the patients, i.e.: DME vendors, Home Health Agencies, Area Aging Office.

Position Details:
The 1-year apprenticeship offers 144 hours of didactic training, 2000 hours of on the job training, and has quarterly milestones. At the conclusion of the 1-year apprenticeship, the employee will be eligible to be certified as a Community Health Worker and transitioned from the Community Health Worker Apprentice to Community Health Worker.
Hours: 8:00a-4:30p- 4 days per week; one late day 9:30a-6:00p
Weekends: None
Location and Travel: Community-Based position located in Pennsylvania. Position requires 50% travel for outreach to patients in Lancaster/York Counties. Use of company car after completing orientation.
Education: High School Diploma or GED is required
Driver's License: Valid Driver's License is required
