Job description

Salvo health is looking for an experienced part time LCSW/LICSW to play an integral role on our care team. 

This role will work directly with patients, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You’ll serve as one of our patients’ primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability.  If you have a passion for patient empowerment through a holistic-based approach to medicine, come join our team!

Responsibilities
  • Conduct comprehensive psychosocial assessments of patients with to identify needs, strengths, and challenges
  • Provide exceptional counseling, disease management and health education to patients
  • Develop and implement personalized care plans in collaboration with patients and an interdisciplinary healthcare team, focusing on behavior modification, coping strategies, and lifestyle changes
  • Support goal setting for individual patients synchronously and asynchronously to help them better manage their chronic conditions and associated mental health needs
  • Conduct regular outreach to patients, based on their needs and preferences, to support engagement and elicit behavior change
  • Document patient interactions, progress, and care plans accurately and promptly in the electronic health record (EHR) system
  • Coordinate with clinical team and advocating for patients to provide an exceptional patient experience
  • Develop and maintain professional, support-oriented working relationships with team members, including participation in case conferences, team meetings, and continuing education activities
  • Offer crisis intervention and support for patients experiencing acute emotional or psychological distress; connecting with local resources, as required
  • Create and revise consumer-facing content advising on mental health best practices for chronic GI conditions and weight management
  • Translate complex medical ideas into digestible messages for a lay audience, including performing research as required to ensure understanding
  • Work with clinical team and outside advisors to draft policies and protocols to ensure consistency of LCSW care delivery
  • Work with a cross-functional product team to develop and constantly improve our in-app patient experience

  • Requirements
  • LCSW license with licensure to practice in NY or MI (also open to PA, FL, LA, OH)
  • 2+ years experience as a an LCSW
  • Bachelor’s and Master’s degrees required 
  • Experience communicating with patients asynchronously preferred
  • Experience with GI conditions or weight management a plus
  • Proficiency in using telehealth platforms and electronic health record (EHR) systems
  • Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease
  • Experience working with patients in a clinic setting preferred
  • Demonstrates a willingness to go above and beyond normal responsibilities to provide the best care possible
  • Excellent interpersonal communication and problem-solving skills
  • Exceptional organizational skills
  • Experience and ability to thrive in a fast-paced, high-growth environment
  • Strong personal computing skills - remote work experience preferred
