Responsibilities

  • Meets with care center leadership of assigned care centers to assess the bereavement services that would be beneficial to the patient's plan of care to meet the community, patient and family needs.
  • Develops an individualized patient bereavement care plan.
  • Conducts a post-death bereavement risk assessment and develops an individualized bereavement plan of care that reflects the needs of each bereaved.
  • Directs bereavement care plans including direction of local clinical staff and regional bereavement program support to implement bereavement care plans.
  • Provides support and outreach to individuals grieving in the local community and provides referrals to community services.
  • Provides feedback on impact of bereavement program impact to identify opportunities to improve program in response to feedback and changing needs.
  • Collaborates with regional manager of bereavement services and local clinical leaders to review bereavement program activity metrics and outcomes and helps develop action plans for areas which need improvement.
  • Engages with local and regional communities and organizations for purposes of bereavement outreach and education.
  • Performs other duties as assigned.

Qualifications

  • Bachelor's degree in social work, psychology, counseling and/or related field.
  • One year experience in family and/or individual counseling with additional training in grief and bereavement.
  • Knowledge of grief and bereavement counseling; understanding of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.
  • Must be familiar with general use and functions of computer.
  • Self-manage online HR services and training.

Preferred

  • Master's degree in social work, counseling, theology or related field or pastoral education with documented evidence of training and experience in grief therapy and bereavement counseling.
  • Three years' experience in family and/or individual counseling with additional training in grief and bereavement.
