Summary of Job Duties:
The Education Specialist/Care Coordinator at the Psychiatric Research Institute Odyssey Clinic supports the NAVIGATE program’s vocational and educational goals for clients experiencing a first episode of psychosis. They conduct assessments, provide job and educational support, and coordinate follow-up services in collaboration with clients, families, employers, and academic professionals. Responsibilities include case management, supporting treatment plans, and coordinating services under the guidance of the Social Work Program Manager or Medical Director. The role involves office, home, and community visits, and requires adaptive technology skills, policy awareness, and participation in team meetings, supervision, and ongoing training.

Minimum Qualifications:
Bachelor's Degree in Social Work or Related Field
Two (2) years of behavioral health or other health care experience
Mental Health Paraprofessional certification and Successful completion of the UAMS CPR Certification within 90 days of hire
Ability to pass UAMS Vehicle Safety Program background check

Works to educate patients and their families in a variety of settings.
Coaches patients on managing their health on a individual basis.
Collaborates with patients to establish health improvement plans and set personalized goals, and supports them in achieving those goals.
Develops educational initiatives for patients and families about their specific conditions and the best way to treat and manage them.
Serves as member of the multidisciplinary Complex Care team to identify high-risk patients and assists with the management of their chronic conditions.
Assists providers with identifying patients due for Medicare Wellness visits.
Follows up with patients who miss appointments.
Works with patients with low health literacy to improve understanding.
Partners with community organizations on educational events and workshops.
Builds and maintains positive relationships in the community and with other service providers.
Participates in community-wide groups and meetings as required.
Supports clinic with various administrative and organizational projects such as Food Pantry, Social Media or other tasks as requested.
