RN - Hospital at Home $20/K BONUS

DAY SHIFT 8a-6:30p

7/10's per pay period (Rotating weekends, rotating holidays)

Responsibilities

  • Performs patient assessment and provides care.
  • Educates patient on their condition, prescribed medications, nursing
  • Interventions and care plans.
  • Participates in the coordination of services with other disciplines and community resources as needed.
  • Identifies and acts on changes in the patient's condition.
  • Evaluates and revises care plans/assessments based on changes in patients and/or their environments.
  • Documents all assessments and interventions.
  • Collects specimens as required (including phlebotomy).
  • Administers medications including IV medications as ordered.
  • Utilizes and instructs patient/family on the use of the telehealth tablet used for biometric data.
  • Participates in regular case conferences and in-service or special case orientations as needed and participates in on-call rotation.
  • Performs other duties as assigned.

Qualifications

  • Associate science in nursing required, BSN preferred.
  • Currently licensed to practice in the state in which you are required to work.
  • Two years of RN experience, three to five years of inpatient RN experience in med/surg preferred.
  • Experience providing RN services in the home, community or clinic is preferred.
  • BLS.
