Job Overview

We are thrilled to offer an exceptional opportunity for a Licensed Telehealth Therapist to become an integral part of our innovative Care Team. Key Responsibilities

  • Evaluate and diagnose both chronic and acute behavioral health conditions.
  • Collaborate with healthcare teams to develop personalized treatment plans tailored to each member’s specific needs.
  • Integrate a behavioral health perspective into comprehensive care planning for patients with complex conditions.
  • Conduct initial assessments and deliver therapeutic interventions, including individual, family, and group therapy, while facilitating referrals to community resources for enhanced behavioral health services.
  • Engage with primary care providers, psychiatrists, social workers, and other healthcare professionals to review treatment plans and progress.
  • Educate members on mental health promotion and disease prevention.
  • Support members requiring additional specialty behavioral health care with advice on treatment plans until they transition into appropriate care.
  • Establish trusting relationships with patients to ensure that care aligns with their personal goals and values.
  • Participate in multidisciplinary case conferences to provide comprehensive and suitable patient care.
  • Educate and coach team members on addressing patients with behavioral health issues.
  • Facilitate post-discharge planning to minimize readmissions.
  • Maintain timely and accurate documentation in our electronic medical record system to oversee member care.
  • Contribute to the design and execution of clinical pathways and protocols for managing behavioral health conditions.

Required Skills

  • 2+ years of experience in outpatient settings with adolescents and adults, specifically those enrolled in Medicaid, Medicare, or dual-eligible programs.
  • 3+ years of direct clinical experience in assessing, planning, and managing behavioral health care, including therapy and case management.
  • 3+ years of experience working with individuals facing significant mental and chemical health challenges, demonstrating a belief in the potential for recovery.
  • 1+ years of experience in telemedicine practice.
  • Familiarity with underserved populations experiencing socioeconomic barriers to healthcare.
  • Solid knowledge of trauma-informed and evidence-based psychotherapy methods.
  • Ability to collaborate effectively within an integrated behavioral health model alongside Community Guide team members.
  • Proficient in electronic medical record documentation and excited about leveraging technology to improve care delivery.
  • Capacity to make prompt and accurate clinical assessments while handling challenging situations with composure and fairness.
  • Strong teamwork abilities, working collaboratively with patients and healthcare providers.

Qualifications

  • Valid license as an LCSW, LMFT, or LCPC; MFT and CPC interns will also be considered.
  • Minimum age of 18 years.
  • Adherence to all federal, state, and local regulations concerning patient confidentiality and HIPAA.
  • Experience with electronic medical records and technological tools.
