Job Overview

Join our esteemed not-for-profit hospice and palliative care organization as a Hospice Registered Nurse (RN) Case Manager in a remote capacity. Key Responsibilities

  • Provide coordinated care to patients and their families, ensuring comprehensive support.
  • Perform detailed nursing assessments to identify the specific needs of patients and caregivers.
  • Design, implement, and assess care plans to guarantee continuity of care and achievement of healthcare goals.
  • Empower patients and caregivers to preserve independence.
  • Engage in interdisciplinary team collaborations and participate in care meetings to enhance patient outcomes.
  • Maintain accurate and timely clinical documentation in line with established best practices and regulatory standards.

Qualifications

  • Diploma or Associate Degree in Nursing is required; a Bachelor's Degree is preferred.
  • An active and unrestricted multistate RN license is mandatory.
  • Current CPR certification is required; CHPN certification is an advantage.
  • A minimum of one year of experience in inpatient settings, along with two years in other nursing roles, is needed.
  • Experience in hospice, palliative care, geriatrics, oncology, family medicine, or home health care is beneficial.
