Job Overview

Join a dedicated and compassionate team of healthcare professionals whose mission is to enhance the quality of life for seniors. We are currently searching for an experienced RN Case Manager to engage with our Senior Care Options (SCO) plan in a remote capacity, facilitating both in-home assessments and telephonic evaluations.

Key Responsibilities

  • Conduct timely initial and ongoing assessments for geriatric patients.
  • Work collaboratively with interdisciplinary teams to devise and implement personalized care plans tailored to each patient’s unique needs.
  • Maintain continuity of care through effective communication with patients, their families, and healthcare providers.
  • Travel within the designated area north of Boston to meet clients as necessary.

Required Skills

  • Valid RN License.
  • Strong assessment, communication, and organizational skills are essential.
  • Familiarity with Medicare and/or Managed Care is an asset.

Qualifications

  • A minimum of 5 years of nursing experience, with at least 1 year in RN Case Management.
