Job Overview

We are in search of a committed and experienced Case Manager II to join our dynamic Case Management Department. This remote position offers the opportunity to significantly impact the lives of individuals receiving waiver services. You will be instrumental in verifying eligibility, crafting and overseeing Service Plans (SP), and ensuring effective delivery of support services tailored to individual needs.

Key Responsibilities

  • Manage a caseload of individuals receiving waiver services, ensuring meticulous documentation is updated quarterly and annually.
  • Organize and facilitate interdisciplinary team (IDT) meetings to align services with individual requirements, promoting effective teamwork and communication.
  • Provide case management assistance to temporarily assigned individuals and support departmental coverage as required.
  • Deliver training and mentorship to Active Case Managers, fostering professional development and growth within the team.

Qualifications

  • Education: A Bachelor's Degree in a relevant field or five years of experience in Long-Term Services and Supports (LTSS) focusing on Developmental Disabilities; a combination of education and pertinent experience may also be considered.
  • Experience: Demonstrated excellence in service coordination or case management, showcasing initiative, leadership, adaptability, and problem-solving skills.
  • Licensure: A valid Colorado motor vehicle license and proof of motor vehicle insurance.
  • Transportation: Candidates must meet agency driving requirements, with potential waivers available upon request.
