Job Overview

As a Case Manager / Patient Support Navigator, you will be instrumental in delivering essential support to patients and their caregivers. Your primary objective is to serve as a reliable resource for Social Determinants of Health (SDOH), guiding patients and their families throughout their journey towards accessing necessary assistance.

Key Responsibilities

In this pivotal position, you will:

  • Act as a patient advocate and educator, providing caregivers and patients with a consistent point of contact for support.
  • Manage incoming inquiries from patients, caregivers, and providers, efficiently triaging and directing them to the appropriate teams when necessary.
  • Share trustworthy information and resources to assist patients in gaining access to essential support.
  • Assess patient needs during initial interactions and throughout the navigation process, connecting them with relevant resources and program offerings.
  • Conduct introductory calls for newly registered patients, offering support materials and vital resources.
  • Maintain compliance with established standards, policies, and procedures.
  • Evaluate barriers faced by patients based on SDOH, including socioeconomic factors, and refer them to suitable resources.
  • Identify local SDOH resource outlets and facilitate scheduling access to these resources.
  • Document incoming data related to patient support programs, ensuring accuracy in record-keeping.
  • Perform benefit investigations and obtain prior authorizations with payers.
  • Collaborate with colleagues to ensure patients receive timely services.
  • Contribute to ongoing departmental initiatives aimed at enhancing patient-centered excellence.
  • Complete various tasks and projects as assigned, including client inquiries and reconciliation reports.

Required Skills

Qualifications

Ideal candidates will have:

  • A Bachelor's Degree along with a minimum of 5 years of experience in healthcare practice management or in a patient services setting.
  • Previous experience in patient support and access, with a preference for call center experience. Familiarity with patient assistance, reimbursement, oncology, medical coding, and payment processes is highly desired.
