Job description

The Company

iAgeHealth is an innovative solution developed by Clinicians in Aged Care, who recognised rural communities fail to have adequate access to multidisciplinary clinical care in a timely manner. Our platform has been designed to harness the latest technological advances and tools to improve access to such crucial health services for older Australians living in regional, rural and remote communities.

With diagnostic tools offering live data to clinicians, this role enables you to provide professional care advice from anywhere, at any time. Our Clinicians and Allied Health professionals work on rotating rosters to support a 24/7 service model. Due to the demand of iAgeHealth, our team is expanding and we are welcoming new professionals to be a part of the solution elderly residents need in our rural communities.

About The Role

We are seeking a highly compassionate Registered Nurse to join our virtual team in an Advanced Care Planning and Palliative Care role. Reporting directly to iAgeHealth Manager, you will be an integral part of supporting our residents and their families to make informed decisions around their future health care needs and desires. Utilising a person-centred framework, you will provide best practice care and advice to elderly residents, provide mentoring and leadership to the broader care team (both virtual and on the ground), and ensure that high quality outcomes are always delivered. This role will see you providing all aspects of palliative care from symptom and pain management, through to advanced care planning. Your compassionate approach and the ability to have open and honest communication with all stakeholders, will ensure that our residents in rural and remote areas are receiving the best quality of life they deserve.

This role is a Fulltime work from home position.
