The Role:

The Network Partnerships Manager will be responsible for developing, managing, and maintaining strategic partnerships that fill service area gaps in our healthcare model. This role builds collaboration between external partners and internal teams, ensuring all partnerships are set up for success, efficiently managed, effectively deliver services, and are compliant. The Network Partnerships Manager will also maintain critical tools, such as the zip code look-up tool and service area maps to ensure accurate service area data and facilitate smooth partner onboarding and support processes.

Responsibilities

Partnership Development & Management

    • Develop and establish new partnerships to fulfill home care service needs in key locations
    • Collaborate closely with the Growth team and Partner Success to identify gaps in in-home services coverage and develop strategies to address them
    • Initiate new relationships with potential partners, negotiate contracts, and onboard partners to CMS with tools already developed
    • Manage existing partnerships through weekly or bi-weekly meetings, addressing partner inquiries, and ensuring smooth communication and collaboration
    • Prepare and deliver training materials, webinars, and resources to support partner success
    • Track service delivery and perform QA to ensure partner Service Level Agreements are met and contract terms are maintained.

Cross-functional Collaboration

    • Collaborate with Growth and Partner Success to understand service area needs and identify new partner opportunities.
    • Work closely with the Guide Program Manager to submit partner rosters and ensure proper documentation of service areas.
    • Coordinate with the Clinical and Patient Engagement teams to ensure zip codes are updated to reflect current service access and address any issues.

Home Care Partner Support & Success

    • Track home care partner relationships and manage the status of partner contracting and onboarding to systems.
    • Provide ongoing partner support post-onboarding, to support training, managing meetings to address any program or service issues.
    • Serve as the main point of contact for partner inquiries, facilitating smooth communication between partners and internal teams.
    • Work with existing partners to onboard new offices as needed and ensure they are set up in CMS.
    • Oversee partner invoicing, ensuring reconciliation and timely approvals.

Zip Code Management & Data Accuracy

    • Ensure any changes to service area zip codes are reflected accurately in the zip code database tool.
    • Collaborate with the GUIDE Program Manager to submit and maintain accurate partner rosters and zip code lists for CMS.
    • Maintain the accuracy of the zip code tool, ensuring updates from partner management activities are promptly reflected.

Qualifications

  • Bachelor’s degree in healthcare administration, business, or a related field
  • 5+ years of experience in partner management, business development, provider relations or healthcare operations
  • Strong ability to build relationships and manage external partnerships
  • Home care agency experience preferred
  • Excellent organizational and project management skills, with experience in managing data tools (e.g., zip code database)
  • Familiarity with healthcare partnerships and CMS onboarding processes is a plus
  • Strong communication skills, both written and verbal, to facilitate partner meetings, training, and ongoing collaboration
  • Strong organizational skills and attention to detail, with the ability to manage multiple projects simultaneously
  • Ability to work collaboratively in a fast-paced environment
  • Knowledge of healthcare systems and payer-provider dynamics preferred

Pay Range Details

The pay range(s) below are provided in compliance with state specific laws. Pay ranges may be different in other locations. Exact compensation may vary based on skills, experience, and location.

Role: Range is $90,000 - $110,000, depending on experience

Bonus: Opportunity to earn up to 10% of total compensation
