About the Role: 

 The Senior Associate, Care Team Operations will play a key role in supporting members in social and care management needs. This position serves as a lead in administrative tasks and follow up for connecting members to their needs, by direction of the assigned Community Health Partners and Nurse Care Managers to the member.  This role will be accountable for ensuring all tasks assigned are completed. 

This position will report to Lead or Manager of Care Teams and is a key member of the Cityblock care team and plays an integral role in ensuring delivery of a state-of-the-art experience to Cityblock members across all teams, performance management for all Operations teams and execution of all operational metrics.

Responsibilities

  • Support administrative and operational functions of a care team, as an acting member of that care team and liaison to hub team 

  • Serve as a representative of Cityblock to members, other health care organizations, practices, insurance partners, and other partners in ensuring that our members have the care and assistance they need to thrive. 

  • At the direction of the Community Health Partner Lead, work closely with team members to coordinate care as a team on behalf of members. 

  • Weekly tasks could include:

  • Coordinate home health, insurance, vendors, and other social service assistance services for our members 

  • Connect with insurance, other providers, and other organizations to request and deliver appropriate medical services and records for members

  • Support team meetings with note-taking and follow-ups on all commitments 

  • Coordinate & communicate team calendar 

  • Use our analytical platforms to generate and communicate weekly reporting & planning to CHP Leads 

  • Over time, learn to analyze reports and help team to plan and learn from data 

  • Use member benefits platforms to organize medicaid support and enrollment 

  • Manage logistics (rooms, food, planning) for case conferences, development sessions, meetings and team outings

  • Experience and empathy with high-complexity Medicaid and Dual Eligible populations  

  • Significant experience and familiarity with ambulatory clinical practice operations serving Medicaid and Dual Eligible individuals 

  • Experience owning and managing a P&L in an ambulatory healthcare setting

  • Comfort using a variety of technologies and systems, including practice management, care management, electronic health record, and billing systems

  • Associates degree required, Bachelor’s degree preferred

  • 2+ Years of previous experience in a Healthcare Operations role
