Position Summary

The Care Coordinator provides support to patients by working with them, the clinical team and other staff to identify needs and facilitate connections to resources, support and services available in the community. The Care Coordinator maintains open communication between all parties involved in patients’ treatment to ensure the highest quality of care is obtained with close collaboration and minimal interruptions in care. The Care Coordinator works with patients to identify their own individualized goals. Using community resources and individualized treatment plans, the Care Coordinator develops a patient-centered plan and approach. The Care Coordinator utilizes tools assigned by the Clinician and Operations Leads to create and activate a patient-centered Treatment Plan. Ultimately, the Care Coordinator role is to ensure patients are receiving holistic support and that all parties involved in patient care are cohesively collaborating.

Position Responsibilities

  • Collaborates with patients, providers, ancillary departments within the company, and community agencies to maintain accurate and timely information exchanges between all entities involved in the patients’ care
  • Provide Care Coordination services to patients; making referrals to appropriate community resources to assist patients in dealing with the barriers they face in areas such as housing stability, legal challenges, access to transportation, physical health needs, economic stability, community integration and access to social support, leisure, and recreation, etc.  
  • Meets with Array patients as assigned to welcome and orient them to the program as well as identify needs, strengths, goals and barriers
  • Facilitates groups on topics relevant to patients served
  • Obtain appropriate Releases of Information (ROI’s) for each community referral that is completed and conduct follow-up for referrals made to resource
  • Assist patients in coordinating community referrals, scheduling and attending appointments, and securing transportation; Address barriers for attending Array appointments
  • Facilitate placement for alternative or higher level of care as agreed upon with the Interdisciplinary Team; Maintain contact with identified placement sites to provide continuity of care upon discharge  
  • Maintain effective communication with patients, providers, team staff, and ancillary departments within the company and community 
  • Enter all required data into all applicable Array systems in a timely and accurate manner
  • Maintain excellent level of documentation and note keeping
  • Understand and adhere to compliance with provisions regarding patient confidentiality
  • Attend virtually regular meetings, including but not limited to Interdisciplinary Teams and external constituent meetings, completing all required documentation in Array systems
  • Provide crisis intervention as necessary
  • Develop a community relations strategy to include regularly engaging outside resources
  • Develop rapport with patients through supportive, treatment-plan-based conversations
  • Maintain individual schedule of assigned patients
  • Other duties as assigned


Position Requirements

  • Bachelor’s Degree is strongly preferred
  • 2+ years experience in a Case Management, or Care Coordination setting
  • 1+ years experience in crisis management/de-escalation
  • Proven experience navigating and documenting in an EMR system
  • Knowledge of assigned market community resources, networking, and referral processes
  • Experience in group facilitation
  • Excellent written and verbal communication, organizational, and customer service skills
  • Ability to work independently with minimal supervision
  • Mental stamina for problem solving, including managing stresses of a mental health service program
  • Ability to prioritize multiple tasks, handle multiple tasks simultaneously and prioritize based on severity of need
  • Ability to document, recall, and explain detailed training, conversation, or events, and apply oral/written instructions, as needed
  • Ability to speak and hear sufficiently to communicate in various forms of spoken and written communication including virtual/telemedicine service delivery


Position Logistics

  • 100% remote position
  • Availability requirements 630a - 330p PT
  • Reports to the VP, Operations AtHome Services
  • Full-time employment
  • FLSA Status non-exempt, hourly
  • The pay range for this role is $18 to $22 per hour. This information reflects the anticipated salary for this position using current national data. The final offer will be determined based on an individual’s skills, experience, education, and other job-related factors permitted by law.
