Your missions

An organization dedicated to enhancing the quality of life for seniors and their families seeks a compassionate and experienced Geriatric Care Manager to join their team.  

In this role, the Geriatric Care Manager will provide personalized care plans and support to elderly clients, ensuring their needs are met with empathy and professionalism. You’ll implement optimal support and provide creative solutions that address clients’ physical, social, and emotional well-being. You will provide clients services and help cultivate new clients in a given area. The Care Manager needs to be able to manage and resolve questions to get the right information. They must know how to build long-lasting relationships, identify issues, and gain client trust quickly.

What You'll Do
  • Advocate for clients and their families, ensuring their needs are met throughout the care process. 
  • Collaborate with a multidisciplinary team to provide appropriate services and high-quality care coordination. 
  • Conduct comprehensive assessments to identify client needs and develop goal-oriented care plans, updating them as necessary. 
  • Communicate regularly with family members, navigating family dynamics while advocating for the client. 
  • Maintain extensive knowledge of community resources available to older adults and provide recommendations for in-home caregivers or residential staff. 
  • Follow all processes, procedures, and systems established by About Senior Solutions. 
  • Act as a liaison for families at a distance and assist with transitions between care settings. 
  • Review financial, legal, or medical issues and refer clients to approved experts when necessary. 
  • Provide crisis intervention, client and family education, and monitor client well-being through regular visits. 
  • Coordinate medical appointments transportation, and ensure clients have access to necessary services in a safe environment. 
  • Manage communication and care plans throughout the conservatorship process, including support for placement in assisted living facilities or nursing homes. 
  • Develop long-range care plans for clients who do not currently need care, identifying available agencies and social services. 
  • Maintain confidentiality of client information and document all interactions in the client management database. 
  • Enter billable hours daily and forecast monthly billable hours for the office manager. 
  • Represent and market About Senior Solutions to create goodwill within the community. 

  • What You Need
  • 2-3 years prior experience as a geriatric care manager. 
  • A graduate degree in a relevant field, such as MSW, RN, MFT, MA in Gerontology, MA in Psychology, or MA in Occupational Therapy, is highly desirable. 
  • Minimum 2+ years prefer 5 years experience post degree in gerontology,  healthcare, behavioral health, psychiatric or RN. 
  • Experience in care or case management of older adults and dementia care.
  • Available Mon-Fri during business hours plus a weekend on-call rotation every 6-8 weeks
  • Knowledge of local eldercare resources (preferred) 
  • Experience working with mental health issues and coordinating services for clients with mental illness
  • Experience working with disabled adults and coordinating services for clients under 65 with disabilities
  • Experience working with elder abuse matters and reporting
  • Proficiency in Google suite of products, Microsoft Word, Excel, Outlook
  • Valid California driver’s license, insurance, personal transportation
  • Strong organization, time management, communication skills, and excellent communication skills (written and oral)
  • Knowledge of utilization management procedures, aging,  mental health, developmental disabilities, and substance abuse community resources and providers. 
  • Knowledge and experience in inpatient and/or outpatient settings. 
  • Masters in:  Gerontology, Behavioral Health, Counseling, Social Work or an RN.

  • Salary & Benefits
  • Location: Remote in San Fernando Valley or San Gabriel Valley, CA. 
  • $30 - $36/hour (Will convert to salary after 30 days)
  • Healthcare: Medical, dental, vision (company contributes partial amount)
  • Vacation: 2 weeks after one year of employment
  • Sick time: 40 hours/year (usable after 90 days)
  • Retirement: 4% 401(k) match (after 1 year)
  • Primarily weekday work
  • Occasional weekend availability for emergencies (ad-hoc basis)
  • No set on-call rotation is currently in place
