Job Description

Assists the Manager or director in planning, organizing, and directing of inpatient Case Management services. This position supervises the RN/LVN Case Managers and support staff that are responsible for providing quality care to our members in acute care or skilled nursing facilities. Responsible for the day to day operations, staffing, utilization review, and discharge panning. Functions as a clinical resource to the staff and as the liaison between the departments. Responsible for basic human resources functions.

Responsibilities

  • Daily supervision of staff members by appraising, monitoring, and observing performance to ensure compliance and adherence of company and department policies and procedures.
  • Ensures continuity of quality patient care by planning daily staff assignments through evaluation of census, patient acuity, and staff competency.
  • Mentor and motivate staff members to excel in their day-to-day performance. Through educational trainings, routinely perform staff member recognition such as awarding company star cards and Employee of the Month nominations.
  • Assist in meeting financial targets by effectively assisting in managing utilization, and productivity, attend daily rounds and assisting with complex discharge planning and solve identified problems.
  • Routine analysis for compliance through daily checks of the case error report, denial processes, closure of pended cases report, and weekly random chart/case documentation audits.
  • Performs the Human Resources (HR) processes for direct report employees by participating in the hiring and interviewing process; completes time and attendance cards; completes the biannual performance evaluations; performs disciplinary processes and advises the Manager/Director of all final written warnings and terminations.
  • Provides coverage as a team resource during staff shortage by assuming a case load.
  • Assists Manager and or Director with the development, implementation, and evaluation of department goals, objectives, policies, procedures, and standards of care which contribute to the achievement of departmental objectives.
  • Performs other duties as assigned.

Qualifications

Minimum Education: Graduate of an approved nursing program required. Bachelor’s degree in Nursing is preferred required.

Minimum Experience: Two to three (2-3) years Inpatient case management experience in IPA/HMO or acute care required. One (1) year experience in supervisor/lead/charge nurse position required.

Req. Certification/Licensure: RN - Current State of California Registered Nurse license required or LVN- Current state of California Licensed Vocational Nurse required. ACM/CCM - Accredited Case Management Certification or Certified Case Manager certification preferred.
