The Opportunity:

This dynamic exciting position allows the Oncology pharmacist to provide holistic care to meet the emotional, psychological, informational, and practical needs of patients while supporting Oncology clinics across Canada. Your role will include navigating the healthcare system and reimbursement through our HAVEN provider solution program. This role will be key at breaking down barriers for patient access to life changing medicines and supporting optimal patient care. The Haven Clinical Specialist (Pharmacist) is responsible for managing patient care, will act as a primary point of contact to their assigned health care providers (HCP) and ensure patients are provided easy access to the prescribed therapy.

The ideal candidate is one who is passionate about patient care and is someone who wants to build out processes that improve efficiencies. The candidate will embrace curiosity and be bold in building best practices that will create significant change in the way patients receive care.

A Day in the Life (What you will do here):

  • Work as a partner with the HAVEN provider solutions team to support HCPs and patients as a Single Point of Contact (SPOC).
  • Perform HCP and patient enrollment activities, according to protocols and applicable departmental procedures.
  • Effectively manages telephone practice of patient/family that demonstrates sensitivity
  • Provide medical support to patients and HCPs, including but not limited to education on cancer, therapies, and disease management.
  • Implements a holistic plan of care that is communicated and clarified with the patient/family.
  • Prepare for and attend extensive, specialized training sessions for specific therapies.
  • Virtual support (including adherence support both inbound and outbound as needed).
  • Review patient files prior to treatment to capture medical history.
  • Utilize pharmacist judgement to interpret and organize sequencing of diagnostic tests and procedures.
  • Documents patient history, physical examination data, care plan and interventions, and the clients’ responses in the client’s record in a timely manner in accordance with the professional standards
  • Maintains confidentiality of patient and corporate information and discusses same only with appropriate personnel.
  • Liaise with physicians’ offices, pharmacies, services providers, and other parties to provide comprehensive and coordinated service to patients and HCPs.
  • Participates in ongoing internal and/or external continuing education activities.
  • Participates in telephone consultations and virtual meetings/training sessions as required.
  • Adhere to professional pharmacy standards at all times.
  • Work collaboratively within multidisciplinary health care team providing oversight.
  • Proactively offer support and advocacy to patients and HCP which includes but is not limited to enrollment, facilitation of coverage renewals, and changes throughout the patient journey, as well as facilitating reimbursement support to physicians and patients.
  • Provide ongoing support on disease state or product related questions.
  • Read and review all Canadian product monographs to ensure a comprehensive understanding of each product being administered.
  • Report and document Adverse Events as per training per Pharmacovigilance requirements.
  • Adhere to Sentrex policies and procedures.
  • Assist in the development and adherence to forms, policies, and work instructions, to ensure the safe handling, preparation, and administration of product.
  • Follow and abide by all provincial College of Pharmacy guidelines while providing patient care.
  • Additional duties as assigned by the manager.


What you need to ensure you are set up for success:

  • Licensed Pharmacist.
  • Minimum 3-5 years’ experience in an Oncology Clinical setting
  • Recent oncology experience preferred.
  • Passionate at supporting patient care and removing barriers to care
  • Bilingual English and French an asset but not required
  • Confident and experienced with using MS Office suite: Excel, Outlook, and Word
  • Ability to work autonomously with little supervision, as part of a larger remote team
  • Experience in Patient Support Programs is a strong asset.
  • Understanding of the pharmaceutical industry is an asset
  • Excellent interpersonal skills
  • Strong organizational skills and attention to detail
  • Ability to meet deadlines and be flexible with patient and HCP requirements
  • Excellent problem-solving skills: ability to resolve issues effectively and efficiently.
  • Excellent verbal and written communication skills
