As a Health Coach, you will play a crucial role in supporting chronically ill patients, many of whom are homebound, on their journey to improved health and well-being.
Location: Various Coverage Areas including Conway Homes, Conway Main Clinic, Singleton Ridge Clinic, and Loris Clinic.
Schedule: Monday to Friday, 8:00 am to 5:00 pm. **
Responsibilities**
Collaborate closely with the patient's care team to develop personalized care plans tailored to their unique needs, goals, and challenges.
Provide compassionate coaching and guidance to patients on lifestyle modifications, including diet, exercise, and medication adherence, to enhance overall health outcomes.
Conduct regular assessments of patients' health status using a blend of remote patient monitoring, telemedicine, and in-person visits, ensuring progress and adjusting care plans as needed.
Educate patients on effective disease management strategies, self-care techniques, and preventive measures to empower them to lead healthier lives.
Facilitate communication and coordination among various healthcare professionals to ensure a holistic approach to patient care.
Assist patients in navigating the healthcare system, offering support in scheduling appointments, obtaining referrals, and accessing necessary resources.
Monitor patients' compliance with prescribed treatments and appointments, providing reminders and encouragement as required.
Maintain accurate and thorough documentation of patient interactions, interventions, and outcomes in electronic medical records.
Stay updated on the latest research, guidelines, and best practices in chronic disease management and health coaching to continuously enhance knowledge and skills.
Uphold confidentiality and adhere to ethical standards in handling patient information.
Embrace a flexible work approach, including a combination of work from home, office, and patient visits. **
Qualifications**
Bachelor's degree in a related field (e.g., health sciences, nursing, psychology) or equivalent experience. Certification in health coaching is preferred.
Demonstrated experience in coaching or counseling individuals with chronic health conditions, ideally within a healthcare setting.
Strong understanding of chronic diseases and evidence-based interventions for managing them.
Exceptional interpersonal and communication skills, with the ability to connect with patients from diverse backgrounds.
Proficiency in technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
Collaborative mindset with the ability to work effectively within a multidisciplinary team.
Empathetic and patient-centered approach to care, with a genuine desire to empower individuals to improve their health.
Flexibility and adaptability to meet patients' varying needs in a dynamic healthcare environment.
Knowledge of local healthcare resources and community support networks is advantageous. **
