Position Overview: 


PCG is looking for an Epic Care Healthy Planet Analyst who is responsible for the implementation, configuration, and support of the Epic Care Healthy Planet module within a healthcare organization. This role involves collaborating with various stakeholders, including population health management teams, quality improvement professionals, and data analysts, to ensure the Healthy Planet module aligns with the organization's population health management strategies and goals. The analyst serves as a subject matter expert for population health analytics and provides technical support to end-users. 


Key Responsibilities: 

  • Epic Care Healthy Planet Module Configuration: Configure and maintain the Epic Care Healthy Planet module based on the organization's population health management requirements, including risk stratification models, care management workflows, quality measures, and patient outreach campaigns. 
  • Data Integration and Analytics: Coordinate with internal and external data sources to ensure accurate and timely integration of clinical, claims, and demographic data into the Healthy Planet module. Utilize population health analytics tools and techniques to identify at-risk populations, gaps in care, and opportunities for quality improvement. 
  • Workflow Analysis and Optimization: Collaborate with population health management teams to analyze existing workflows and identify opportunities for streamlining and optimizing care coordination, patient engagement, and population health interventions. Implement changes to improve efficiency and outcomes. 
  • Reporting and Dashboards: Develop and maintain comprehensive reports, dashboards, and data visualizations within the Healthy Planet module to support population health management initiatives. Generate actionable insights and provide data-driven recommendations to stakeholders. 
  • Training and Support: Develop and deliver training programs to educate population health management teams, quality improvement professionals, and other end-users on the use of the Healthy Planet module. Provide ongoing support, troubleshoot issues, and resolve user inquiries in a timely manner. 
  • Regulatory Compliance: Stay updated with relevant regulatory guidelines and requirements related to population health management, quality reporting, and value-based care initiatives. Ensure the Healthy Planet module meets these requirements and assist in the documentation and reporting of quality measures. 
  • Documentation and Project Management: Maintain comprehensive documentation of workflows, system configurations, and changes made to the Healthy Planet module. Manage projects related to module enhancements, upgrades, and new implementations, ensuring adherence to timelines and deliverables. 
  • Continuous Improvement: Stay abreast of industry trends and best practices related to population health management, value-based care, and EHR systems. Recommend enhancements and improvements to the Healthy Planet module to enhance analytics capabilities, care coordination, and patient outcomes. 
Requirements

 

  • Bachelor's degree in healthcare informatics, public health, healthcare administration, or a related field. Relevant certifications in Epic Care Healthy Planet are highly desirable. 
  • Prior experience working with the Epic EHR system, specifically in the configuration and optimization of the Healthy Planet module. 
  • Strong understanding of population health management concepts, including risk stratification, care management, quality improvement, and value-based care. 
  • Knowledge of healthcare data sources, including clinical, claims, and demographic data, and experience integrating data into population health management systems. 
  • Proficiency in population health analytics tools and techniques, such as data modeling, data visualization, and performance reporting. 
  • Excellent analytical and problem-solving skills with the ability to interpret complex data and identify actionable insights. 
  • Strong communication and interpersonal skills to collaborate with cross-functional teams, stakeholders, and end-users. 
  • Ability to prioritize tasks, meet deadlines, and work in a fast-paced environment. 
  • Attention to detail and a commitment to maintaining data integrity and patient privacy.
