Make $90,000 to $95,000 working remotely empowering patients with heart disease to live the lives they aspire to live. You will be responsible for assessing patients behavioral health, developing/managing a personalized care plan tailored to their specific behavioral health needs, and coordinating their care across Our Companys and the Health Systems care team. You will be a part of an integrated team of clinical professionals including Nurse Practitioners and Clinical Exercise Physiologists working together to deliver a world-class patient experience.
Requirements:
Masters Degree or higher (mandatory)
Mandatory Remote/Virtual experience or Digital Healthcare (3+ years)
Illinois LCSW license from an accredited program
Willingness to obtain licensure in additional states
Must work Mon - Fri 8 AM - 5 PM
Evening Availability via Message Platform required
Minimum of 3-5 years experience in practicing CBT
BSc in Psychology, or related field
Experience in Cardiac Health a plus (Preferred)
Must be tech-savvy, passionate, and experienced in psychotherapy
Must have an innovative mindset, and willingness to challenge the status quo of traditional cardiac rehab experience
Job Duties:
Participate in the design and launch of Our Companys comprehensive, evidence-based virtual-first cardiac recovery program
Provide virtual psychotherapy (45-50 min per session) to a panel of patients
Provide therapy for (Depression, Nutrition, Smoking Cessation, etc.) and support a holistic interpretive care model.
Evaluate patient symptoms and risk factors to determine diagnoses and develop individualized treatment plans
Ensure that treatment plans are supported by standard practices and/or current literature
Follow the plan of care to accomplish functions within established policies, procedures, and scope of practice
Re-evaluate treatment plans, as needed, to ensure safety and maximum patient benefit
Promote patient engagement, and successfully achieve patient and program goals
Make independent judgments and changes to treatment according to professional and regulatory standards
Counsel patients using behavioral change techniques (ex. mindfulness, motivational interviewing)
Communicate effectively across Our Companys integrated clinical team and our partner health systems, escalating unusual and emergent situations
Provide complete, accurate, and timely documentation of patient sessions
Interview Process:
1st round - Provide company overview, role overview, clarify expectations and answer basic questions (1 hour)
2nd round - CEO intro and verify company culture fit (30 min)
3rd round - Group interview with situational/behavioral questions and/or role-playing (1 hour)
4th round - optional interview with a Motiv advisor or Straight to offer
