Job description

Job Title: Physician/Nurse Practitioner for Health Risk Assessments

Overview

Medigence Health is actively seeking dedicated and skilled Physicians and Nurse Practitioners to conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members. This fully remote position entails performing comprehensive assessments in various settings, including home visits, clinics, telemedicine, or long-term care facilities. Our mission is to enhance the quality of life for those we serve through positive personal interactions.

Key Responsibilities

  • Conduct thorough evaluations encompassing medical and social assessments utilizing Medigence Health’s tablet-based assessment tool.
  • Identify and refer members for routine, urgent, and emergent case management.
  • Provide recommendations for follow-up care with primary care physicians.
  • Travel throughout local counties and regions where you hold a current license in Wisconsin, including but not limited to: Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Winnebago.
  • Maintain compliance with regulatory standards for Health Risk Assessments.

Required Skills

  • A minimum of 2 years of hands-on experience as a Nurse Practitioner or Physician.
  • Familiarity with the HCC model, HEDIS, and STAR quality measures.
  • Strong communication abilities, particularly with elderly and chronically ill members.
  • Proficient in telehealth technology and mobile devices.
  • Willingness to engage in home visit environments.

Qualifications

  • Current and unrestricted license in the state of Wisconsin or the capacity to obtain one.
  • Registration with Medicare and Wisconsin Medicaid is required.
  • Board certification is preferred, ideally in internal medicine, geriatric, or family medicine.
