Position Title: Physician/Nurse Practitioner - Health Risk Assessments

Location: Remote

Department: Clinical

Travel Requirement: 100%

Reports to: Chief Medical Officer

Job Overview

Medigence Health is seeking a dedicated and experienced Physician or Nurse Practitioner to conduct Health Risk Assessments for members with Medicare, Medicaid, and Commercial Marketplace coverage. This role supports primary care physicians, medical groups, and health plans by delivering high-quality, compliant assessments in diverse settings, including patients’ homes, clinics, via telemedicine, or in long-term care facilities. Key Responsibilities

  • Conduct comprehensive evaluations of the medical and social factors affecting beneficiary members.
  • Identify and refer patients for routine, urgent, and emergency case management services.
  • Recommend follow-up actions with primary care physicians based on assessment findings.
  • Travel throughout local counties, regions, or states in which you are licensed.
  • Perform thorough assessments in provider offices or via Telehealth, ensuring effective audio and video connectivity.
  • Document assessments accurately within the designated EMR system.

Required Skills

  • Minimum of 2 years of practical experience as a Nurse Practitioner or Physician.
  • Familiarity with HCC model, HEDIS, and STAR quality measures.
  • Current and unrestricted license in the state of Connecticut or ability to acquire one.
  • Registration with Medicare is mandatory.
  • Board certification, preferably in internal medicine, geriatrics, or family medicine specialty.
  • Strong communication skills, particularly with elderly and chronically ill patients.
  • Proficient in utilizing telehealth technology and electronic medical records.

Qualifications

  • Must possess a current and unrestricted license in Connecticut.
  • Board certification in a relevant specialty is required.
