Job Overview

Medigence Health is seeking dedicated and licensed Nurse Practitioners or Physicians in Michigan to deliver comprehensive Health Risk Assessments for our Medicare, Medicaid, and Commercial Marketplace patients. In this pivotal role, you will conduct thorough evaluations in a variety of settings, including home visits, clinic consultations, telehealth appointments, and long-term care facilities. Our core mission is to enhance the quality of life for our patients by providing a compassionate and positive healthcare experience.

Key Responsibilities

  • Conduct detailed medical and social assessments utilizing the Medigence Health tablet-based assessment tool.
  • Identify and refer members/patients for routine, urgent, and emergent case management.
  • Provide recommendations for follow-up care with primary care physicians.
  • Travel across local counties, regions, and states within your licensed jurisdiction.
  • Perform comprehensive assessments via Telehealth, ensuring effective audio and video connectivity.

Required Skills

  • A minimum of 2 years of practical experience as a Nurse Practitioner or Physician.
  • Knowledge of HCC model, HEDIS, and STAR quality measures.
  • A current, unrestricted license in Michigan or eligibility to obtain one.
  • Registration with Medicare and Medicaid.
  • Board certification in a relevant specialty, ideally internal medicine, geriatrics, family medicine, or nephrology.
  • Strong communication skills with an emphasis on interacting effectively with elderly and chronically ill patients.
  • Familiarity and comfort with telehealth technology.
  • Proficient in using computers, tablets, and mobile devices.

Qualifications

  • Possession of a current, unrestricted license in the state of Michigan.
  • Board certification is preferred, especially in relevant specialties.
